| Service Description: | The Self Directed Support Team (SDS) are a team of 10 Coordinators that support families and individuals to arrange their own support through a direct payment. The Coordinators work across a range of disabilities from 0 to 25 and over 25’s.
Early intervention: 0 to 17 (inclusive) the SDS team provide funding for families, through a direct payment to enable them to use the money flexibly to give them a short break from their caring responsibilities.
0 to 25 and over: SDS work with families, individuals and professionals to enable families and individuals to develop their own support plan and arrange their own support through a direct payment. The direct payment can be used flexibly and creatively providing it meets the agreed outcomes with the social worker.
