Service Description:Making A Difference is a wrap-around family support service that provides support within the family home and short breaks in the community for children and young people with disabilities and behaviours that might sometimes be challenging.
Short breaks are community-based activities and provide opportunities for young people with disabilities to have enjoyable social experiences away from their parents/carers. In providing professional one to one support, we ensure our children and young people have fun and positive experiences whilst their families take a break from their caring role.
Popular short breaks chosen by our younger ones and their families for example include activities such as swimming, cinema, trampolining, soft play, picnics in the park, dining out, bowling, days out to beaches, zoos, theme parks, roller skating and ice skating.
Alternatively, we can provide support for you and life skills for your child or young person within their own home or even accompany you on family outings so everyone can have an enjoyable shared experience.
Referral/eligibility criteria: self-referral, 0-18 disability team, Children services. Adult services, Short break team, Social worker.
Age Groups:Up to 65 years
