Service Description:Mainstream schools are increasingly supporting pupils with complex SEND, social, emotional and behavioural needs — often without the specialist capacity, time or confidence required to make sustained change. Rising behaviour incidents, persistent absence, escalating parental concerns and pressure to avoid exclusion place significant strain on staff and systems. Schools are looking for practical, specialist support that works alongside them to strengthen inclusive practice and keep pupils successfully learning in their mainstream setting.
Fortis Trust Outreach provides specialist, practical support that helps schools respond confidently to these challenges and improve outcomes for pupils with SEND across Nurseries, Primary and Secondary settings.
Our outreach work is bespoke and responsive, shaped around each school’s context, priorities and pupils. Support is delivered by highly experienced practitioners with a proven track record of securing real, sustainable improvements in SEND provision, behaviour, attendance and staff confidence. We work alongside school teams to embed effective strategies, strengthen inclusive practice and support pupils to succeed within their mainstream environment.
Our team can support specialist settings and provide advice and guidance on the set up of mainstream inclusion bases (SEND units or hubs).
Some elements of our outreach offer are commissioned by Medway Council for Medway schools.
