Service Description

Service Description:
A social care service for children and young people with substantial disabilities within the Medway Area. The team consists of an Area Manager, Senior Social Workers/Practice Managers, Social Workers, Occupational Therapists and Family Workers/Social Care Officers.

The 0-25 Disability Team provide specialist services to a defined group of children and adults under the age of 25. These services include social work support, resources in the local community, support packages including care, and short breaks tailored to meet the individual’s needs.

The service is accessible to people living in Medway who have a substantial and long-term disability and meet the threshold of the eligibility criteria for services and support, provided by the 0-25 Disability Team.

The service completes assessments and provides support to Children In Need, Looked After Children, adults between the ages of 18 to 25 with disabilities, and works in partnership with Medway Council’s Safeguarding Social Work Service.
